How much does the Young Stroke Service cost?

Registration and services provided within the Young Stroke Service are free.
We may refer you to other services do have a cost to you. We will let you know if there might be any cost to you.
You can say no at any time.

How do I get a referral into the Young Stroke Service?

To get a referral you must:

  • download a referral form below or contact the team for a copy. Get a copy of the referral form by going to our For Young Stroke Survivors 🡥 or For Health Professional 🡥 pages or by contacting the team
  • give the referral form to your health professional
  • ask your health professional to complete the form
  • return the referral form back to us completed
Who can refer you to the Young Stroke Service?

You must be referred by a health professional. This can be:

  • your GP (for example a doctor)
  • a health professional in the hospital (for example a neurologist or nurse)
  • a health professional you see in the community (for example a physiotherapist or psychologist)
How do I return the referral form to the Young Stroke Service?

There are different ways you can return the form.
Please return the form using one of the ways below:

This option is only available to people in Victoria. Please fax to 03 9035 7304.

Health Professional
You can ask your health professional to return the referral to us.

Will this service use my National Disability Insurance Scheme (NDIS) funding?

The services from the Young Stroke Service are free and will not use your NDIS funding.

If you have NDIS support, we can help you find NDIS providers in your community. For example, if your plan includes a goal to drive again, we can help you get a driving assessment.

I already had community rehab services, is this not the same?

Our service does not take the place of other community services.

We support people who have finished stroke rehabilitation but still need help. We also help access a specific service.

Am I eligible?

You are eligible for the Young Stroke Service if you:

  • Have had a stroke
  • Are aged 18 to 55 years old
  • Live in Victoria and South Australia
I have just had a stroke, am I eligible?

Yes, you are eligible if you have just had stroke and you are early in your recovery.

You must be between 18 and 55 years old. You also must live in Victoria and South Australia to access the service.

My stroke was a long time ago, can I participate?

Yes, you are still eligible if your stroke happened a long time ago.

You must be between 18 and 55 years old. You also must live in Victoria and South Australia to access the service.

How long can I access this service?

The Young Stroke Service is a trial program that will run until December 2026.

Where are the services located?

We operate an in-person service in Victoria and South Australia. We also provide a telehealth service in both locations.

Victoria
We are at various sites in the Melbourne area, including:

  • Young Stroke Service Clinic - The Florey, Melbourne Brain Centre at Heidelberg
  • Young Stroke Service Neuropsychological Intervention Hub - La Trobe University, Bundoora Campus

South Australia
Our services in South Australia are at Flinders Medical Centre - Bedford Park

Telehealth
You can use telehealth for our services but you still need to live in Victoria or South Australia.

Is there parking available?

Parking is different at each location.
Please check your travel options with our team when you book your appointment.

I live in a regional area. Travelling to Melbourne or Adelaide would be difficult. Can you still help me?

You can use telehealth if you live in Victoria or South Australia.
Please check with the team to see what telehealth options are available to you.

What happens during an intake appointment?

Your intake appointment is with our clinical therapist.
You can do this in-person at the Melbourne Brain Centre in Heidelberg or by telehealth.
The appointment will take about 1 hour to 1.5 hours.

We will talk to you about:

  • your stroke
  • any symptoms you have now
  • treatments you have had in the past

You will answer some questions to help us understand what services or support may help you.
After the appointment, our team will review your information. We will contact you once we decide how we can best support you.

What does the Young Stroke Service do with my personal information?
  • When you join Young Stroke Service you share some personal information.
  • You allow us to collect and use this information. The Young Stroke Service has ethical approval to do this.
  • We use your information to help with your healthcare and to check how the Young Stroke Service is working.
  • We keep your information for at least 7 years and store it safely.
