Our Service

The Young Stroke Service helps people recovering from stroke. We offer assessment, referral and treatment options.

We can also provide specalised guidance about young stroke to clinicians.

We do not replace other services in the community.

Confirming referral

The Young Stroke Service team will contact the referrer to confirm the referral.

Call with the Young Stroke Service

Our first call with the young stroke survivor will tell them what our services do, answer any questions they have and set up their intake appointment.

Intake appointment

The young stroke survivor will meet with our clinical therapist. The therapist will ask them about their stroke, their symptoms, and any treatment they have had.

Our team will discuss what we can offer after the appointment.

The referrer will get a copy of the recommendations.

Refer to the Young Stroke Service

Young stroke survivors need a referral from a health professional to join the Young Stroke Service.

You can use our digital platform to:

  • fill out the online referral form, or
  • download the referral form to fill out offline.
