As a young stroke survivor

Young stroke survivors can use the platform to tell us:

• that you agree to join the service
• how you want the service to contact you
• if you want to hear about research you can take part in, and
• about your unmet needs

Contact us if you are already part of the Young Stroke Service and want to use the new platform.

As a health professional

Health professionals can use the platform to refer patients to us online.
You can refer patients by:

• filling out the online form, or
• downloading the referral form to fill the form out offline
