Using the digital platform

The Young Stroke Service has made a digital platform.
Find out how to use it below.

As a young stroke survivor

Young stroke survivors can use the platform to tell us:

that you agree to join the service
how you want the service to contact you
if you want to hear about research you can take part in, and
about your unmet needs

Contact us if you are already part of the Young Stroke Service and want to use the new platform.

As a health professional

Health professionals can use the platform to refer patients to us online.
You can refer patients by:

filling out the online form, or
downloading the referral form to fill the form out offline
